Reference to the advertisement for the entry test in Doctor of Physical Therapy (DPT in the newspaper daily Nawai e Waqat and The News on dated 24th Nov 2019.
The applications are invited for 25 seat for DPT program in SZABMU/PIMS from the eligible students on following criteria.

Attested photo copies of the following documents must be attached with application form:

1. One Passport size photograph.
2. Computerized National Identity Card (CNIC)
3. Domicile certificate.
4. Matric, F.Sc, certificates or equivalent
5. F.Sc, certificates or equivalent

Procedure for Admissions

1. Application processing fee is Rs. 1000.
2. Applicants will create a Bank Draft/ Pay Order in favor of Shaheed Zulfiqar Ali Bhutto Medical University Islamabad (NTN# 9010030-4). Fee is non-refundable/non-adjustable.
3. Last date for submissions of application forms is 28-01-2020 till 02:30 PM.
4. Application form along with mentioned documents should reach Registrar Office, Admissions & Registration Department, Shaheed Zulfiqar Ali Bhutto Medical University, Islamabad before closing date.
5. University will not consider applications which are incomplete or have not been submitted with the stipulated deadline.
6. In case of incorrect information, the University has the right to cancel the application admission without any fee refund.
