Registration of Postgraduate Residents of MS/MD/MDS for Session July, 2019
Two (2) attested copies of the following documents must be attached along-with form:
1. Three Passport size photograph
2. Computerized National Identity Card (CNIC)
3. Domicile certificate
4. Migration Certificate from Last attendant Institute (In original)
5. Matric, F.Sc, certificates or equivalent
6. MBBS/BDS degree with detail marks certificates of all professional examinations.
7. House Job certificates
8. PM&DC valid permanent registration certificate.
9. MD/MS/MDS Part-I passing certificate
10. Experience Certificates (if any).
11. NOC/EOL (Government employee)
Last date for submission of application forms is 30-09-2019
Registration Fee 16000/- & Registration form Fee Rs 500/- deposit in NBP PIMS Branch.
